Purpose

To recognize and respect the rights of each neuromonitoring patient and to provide the highest quality of care based on the patient’s individual needs.

Patient Rights

1. Each neuromonitoring patient has the right to a reasonable response to his or her needs for treatment and services within the scope of care provided by Alpha Neuro-Monitoring LLC (“Alpha”). Upon admission to each hospital or other healthcare facility served by Alpha, the patient will receive a copy of these Patient Rights and Responsibilities.

2. Each patient is assessed for individualized needs based on age, physical condition, cultural

factors, and other issues. Care is customized to meet these needs whenever possible.

3. Informed Consent: The patient has the right to make decisions involving his or her care. The patient is informed about the risks and benefits of neuromonitoring at the time he or she provides informed consent to the surgical procedure, usually in the surgeon’s office. The surgeon confirms in writing on the History and Physical or Progress Note that an informed consent has been given. The patient has the right to refuse neuromonitoring at any time before the commencement of the surgical procedure.

4.
Patient Privacy and Confidentiality: Each patient’s privacy is protected in accordance with the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rules and each employee or independent contractor of Alpha has been trained regarding his or her HIPAA

obligations. Discussion of the patient’s needs and health information will be held only with those individuals involved in the patient’s care. No information is ever disclosed over the telephone to family or third parties without the patient’s permission.

5. Identification of Neuromonitoring Team: All members of Alpha’s intraoperative monitoring team will inform the patient of their name and the purpose of their care.

6. Effective and Timely Communication: The patient will receive information in a manner that is appropriate for his or her age, primary language and level of understanding.

7. Patient Complaints: The patient is encouraged to express opinions, compliments and complaints at any time.

Patient Responsibilities

1. Patients are responsible for honoring their financial commitments for Alpha’s services.

2. Patients are responsible for observing the rules and regulations of the hospital or facility where they are being treated, including with respect to neuromonitoring services.

3. Patients are responsible for providing monitoring personnel with accurate and complete information regarding their past medical history, if applicable.
